Skilled Home Health Services

Allowing individuals to remain in the comfort of their home as they recover from an acute or chronic illness.

  • Home health is a bridge program between the hospital and home with the goal to stabilize and remain independent
  • Patients on home care services require skilled care for a specific problem - much like an acute care patient being admitted to the hospital for a specific issue
  • This patient will be on service with the intention of them improving and being discharged off service
  • Unlike hospice, we are intending on treating the patient and staving their decline
