Home Care Options

Our comprehensive, individualized approach puts symptom management and quality of life first, allowing patients to continue living their lives at home whenever possible.

The Right Care Where You Need It

Connecticut Hospice provides compassionate and comprehensive home care services for people at any stage of progressive or acute illnesses, from diagnosis through active treatment, to end of life. Flexible care options enable our patients to move back and forth between our home care and our inpatient services over the course of their illness, if necessary.

A physician-led home care team assesses patient needs and re-evaluation as level of disease progresses and needs change, with the goal of symptom management and quality of life always being primary.

Interdisciplinary and Comprehensive Care

In addition to medical care and support, Connecticut Hospice offers patients and their families a wide variety of in-home complementary services to help patients, and their loved ones, live life to the fullest possible. 

Patients, and their families, can receive complementary therapies and services in whatever setting the patient resides.

Comprehensive Support

Some in-home complementary service examples include:

  • Social workers are available to visit patients and families for counseling and guidance with care plans and end-of-life planning.
  • Trained staff and volunteer musicians, visual artists, writers, and other artisans offer arts exposure to music, poetry, writing, visual arts, crafts, drama, movement & more.
  • Our Pastoral and Spiritual Care team helps connect people with their faith, regardless of what it is, calling on spiritual leaders from Catholicism, Judaism, Buddhism, and other religions to offer patients and families the support they need in-home.

Hospice Home Care

Our home care program is designed to meet individual needs and assist patients and their families at different stages in the course of their illness. We provide continuity of care, should illness progress and inpatient hospice care become necessary.

Hospice Care at Home

Our experienced, physician-led interdisciplinary team works together to provide comprehensive care to our patients and families in their own homes, assisted living and skilled nursing facilities, and other hospitals. 

Our Hospice Home Care program is appropriate for patients with terminal illnesses whose life expectancy is less than six months.
