The Connecticut Hospice provides expert palliative and hospice care 365 days a year.
If your patient is ready for hospice home care and lives at home, in a skilled nursing facility, or resides in assisted living, we can provide hospice care. If they need inpatient care, our unique licensed hospice and palliative care hospital is available to them.
We can provide palliative care for patients in their homes, assisted living facilities, and in our own intimate 52-bed licensed hospital situated on the edge of the Long Island Sound.
Our experienced Intake and Admissions staff is available at 203-315-7301 to discuss patient eligibility and care options, explain our intake process and coordinate your referral with your patients and authorized family members or caregivers.
As a not-for-profit, we depend on generous donors to help us provide customized services and therapies that aren’t completely covered by Medicaid, Medicare, or private insurance.
