Short-Term Skilled Intervention for Palliative Patients

Our Acute Care Hospital provides short-term, skilled intervention for patients experiencing acute issues that require prompt and professional medical attention. These patients are expected to stabilize and return home once their immediate needs are addressed.

Key Characteristics of Acute Care Patients:

  • Patients present with a specific medical issue that requires skilled and timely care
  • They are admitted under the specific problem being treated
  • Most patients are also receiving palliative care and have skilled needs such as IV antibiotics, IV hydration for dehydration, wound care, etc.
  • These patients are expected to improve and be discharged back home
  • Documentation practices differ due to the expectation of recovery
  • The main function is to retain patients within our care system, reducing unnecessary transitions between home, ER, and PCP visits
  • These patients are expected to improve and be discharged back home
