Sooner Than You May Think...

Whether you or your loved one is receiving medical care utilizing drugs, radiation, or surgery to treat your disease, or you have stopped treatment and only want comfort care, hospice and palliative care are most beneficial when they are provided for months rather than weeks or days.

Unfortunately, most people don't elect to receive hospice care until the final weeks or even days of life and miss out on months of supportive care and quality time.

To help our patients and their families live life as fully as possible, our interdisciplinary teams create customized plans of care.

These teams consist of physicians, nurses, pharmacists, social workers, spiritual care providers, music and art therapists, and trained volunteers.

The customized plans of care help patients and families maximize quality of life while navigating the challenges surrounding irreversible illness.

Ready Signals

If you're not sure whether you or your loved one are ready for hospice or palliative care, this list of signals might help.

Have you or the patient:

  1. Been hospitalized or gone to ER several times in past 6 months
  2. Needed to visit your doctor more frequently
  3. Started taking medication to lessen physical pain
  4. Started spending most of the day in a chair or bed
  5. Fallen several times over the past 6 months
  6. Started needing help bathing, dressing, eating, getting out of bed, or walking
  7. Started feeling weaker or more tired
  8. Experienced significant weight loss
