Dementia patients and their caregivers can lead a life well-lived with support from the Guiding an Improved Dementia Experience Model (GUIDE) Program and Connecticut Hospice.

GUIDE is a program designed to support Medicare A & B patients with dementia and those who care for them. As one of only two approved GUIDE partners in Connecticut, we are uniquely equipped to be a valuable resource in ongoing dementia care.

In addition, GUIDE provides caregivers with vital education, and helps families seamlessly coordinate with their doctors. Connecticut Hospice is an ideal partner because we understand each patient has unique needs.

That’s why we offer tailored care plans for every patient. It’s also why we provide respite services to caregivers who need a well-deserved break—because Connecticut Hospice treats dementia patients and their families as single units of care, each deserving of physical, emotional, and spiritual support.

With comprehensive support and our dedication to comfort-focused care, dementia patients and their loved ones can receive the personalized support they need to honor their shared journey together.

How GUIDE Helps You Thrive at Home

Dementia patients living in New Haven, Fairfield, or Middlesex counties, CT, can receive free, comprehensive GUIDE support, which includes:

Help maintaining independence and staying at home longer

Connection to valuable local resources to aid patients' care

Vital education for caregivers

Seamless coordination with patients' existing doctors for optimal care

Connecticut Hospice supplements these resources with industry-leading care, which includes:

You May Qualify

Dementia patients and their caregivers may qualify for the GUIDE program if they:

Have a diagnosis of dementia at any stage
Are enrolled in Medicare Parts A and B (not Medicare Advantage plans)
Live in the community (not a long-term care facility)
Reside in New Haven, Fairfield, or Middlesex counties

Before enrolling, patients and caregivers should also discuss the GUIDE program with their primary care physicians or neurologists. Together, they will be able to help determine if it’s a good fit for patients’ needs.
