In 1974, The Connecticut Hospice Inc., a 501(c)(3) not-for-profit, founded America’s hospice movement. Now known simply as Hospice, we have continued to lead the Hospice movement for over four decades, changing and growing to incorporate best-practices, and developing the interdisciplinary team approach that is the hallmark of our services.
As a not-for-profit, we exist in part by the grace of donors and grateful families like you. About The Connecticut Hospice
Since our founding over 40 years ago, thousands of CT Hospice patients and their families from across our state have benefited from our multi-disciplinary team approach and expert hospice care. We currently employ over 200 people, serve patients in over 70 cities and towns, and collaborate with over 30 Connecticut hospitals, skilled nursing facilities and home care providers to provide expert palliative and end of life care 365 days a year.
Anyone with an illness that can’t be cured by medical intervention is eligible to receive our care. We create flexible plans of care that enable patients to move back and forth between our home care and in-patient services. When curative treatment is no longer an option, our interdisciplinary teams work together to manage symptoms, provide comfort and achieve the best quality of life possible for our patients and their families.
We provide palliative and hospice care services to patients in their homes or at our inpatient facility with medically directed, nurse-coordinated, interdisciplinary teams that continue to provide support from diagnosis through bereavement. Together, our physicians, nurses, pharmacists, social workers, clergy, artists, and trained volunteers actively assist each patient and family to navigate the myriad of challenges surrounding irreversible illness. It is our “team” approach – comprehensive, coordinated care without gaps or duplication – that truly distinguishes Hospice within our health care system.
We realize that our patients have many choices these days for home and hospice care, and we are appreciative that so many have chosen us and have benefited from our supportive services through our home care programs and/or our beautiful waterfront inpatient facility. We hope that they will continue to rely on us in the future, and we look forward to providing Connecticut residents with excellent hospice and palliative care going forward, just as we have for the past 45 years.
