This survey, developed by The Connecticut Hospice, Inc. and The Hospice Institute for Education, Training and Research, Inc. (collectively, “Connecticut Hospice”) is designed to assist medical research in the area of bereavement and depression. The reliability of the survey obviously depends on the information supplied by the respondent. Entering incorrect or misleading information will cause erroneous or unreliable results.
THIS SURVEY DOES NOT MAKE A “DIAGNOSIS,” something only a physician can do. The purpose of the survey is to assist medical research. It merely measures the existence of conditions based on its internal logic and your answers to the questions.
It is the opinion of legal counsel that using this survey is not a substitute for medical treatment or diagnosis and cannot be construed as the practice of medicine. Still, we must stress this disclaimer:
CONNECTICUT HOSPICE MAY NOT BE HELD LIABLE FOR ANY MISDIAGNOSIS, DAMAGES, INJURY OR DEATH OCCURRING TO ANY RESPONDENT WHOSE FINDINGS ARE ENTERED HEREIN.
The survey emphasizes a clinical approach, but it remains, nonetheless, A TEST PROCEDURE. Thus, the respondent should contact a physician or mental health professional regarding the outcome of this survey. As always in the practice of medicine
CLINICAL JUDGMENT TAKES PRECEDENCE
THEREFORE, THIS SURVEY SHOULD NEVER BE A SUBSTITUTE FOR SEEING YOUR HEALTH CARE PROFESSIONAL.
